Records
The Records Division of the Stickney Police Department is responsible for the management and coordination of police records, which include processing of Freedom of Information Requests for Police Department related materials.
FOIA
All requests under the Freedom of Information Acts (FOIA) must be submitted in writing (Click here for the form) and may be made either in person at the Village of Stickney Village Hall at 6533 Pershing Road in person (Monday to Friday between 8:00am and 4:00pm and the first and third Tuesday of the month between 8:00am and 7:00pm, except holidays), by mail or via facsimile to 708-749-4451 or via email to foia@villageofstickney.com.
Report Copies
Copies of police incident and accident reports are available at a cost of $5.00 each. An “Authorization for the Release of Reports and Records Form” must be completed and submitted to the Records Division.

Authorization for the release of reports and records form
You must be a party involved in the report, incident or accident, or the legal representative of an involved party (as noted in the Authorization for the Release of Police Reports and Records) to obtain a copy of the report that you are requesting.
Complete the “Authorization for the Release of Police Reports and Records” and submit it to the Records Division in person, by mail, or via facsimile to 708-749-2742, or via email to records@stickneypolice.com. (Click here for the form) Signature of the requestor MUST be included in all requests for records. If you are unable to complete the form online, you can stop by the police department to obtain a copy of the request form. If you come to the police department and fill out the request, there is no guarantee that you will get your copy at that time. We will reply to most requests within 5 business days (average time is 1-2 days).
If the report you are requesting is a criminal or arrest report, it will only be released by the approval of the investigating detective or once the case is no longer an active investigation or prosecution.
Insurance companies can submit a written request for records in lieu of submitting a request form.
Payment must be received before your report/record can be released. Each report is $5.00. Checks should be made payable to the Village of Stickney. Other forms of payment include cash or credit (in person only).
*Once a report has been processed and is ready to be released, you can pick it up at the police department desk Monday –Friday from 7am-11pm.
